I'm suggesting a button or link on each post which allows members to instantly notify moderators of the post, and perhaps provide some kind of description of what they consider the problem.
This functionality could be directly tied to the PM system and send the thread link and report to all moderators, or the moderator of the particular forum.
It's used extremely effectively on other forums I visit. It removes the bias which may come from reading an entire thread, as the moderator is directed to the flagged post and can instantly evaluate it to check it complies with the forum rules. This basically gives the community the ability to police itself, but defaults to a moderator to make the final judgement on the 'legality' of the post.
