Former CIA chief George Tenet failed to prepare adequately for al-Qaeda threats in the lead-up to the 9/11 attacks, according to an official inquiry. The report claims that Mr Tenet was “so busy schmoozing with foreign leaders … that he forgot his job was to manage the intelligence community”.

Could more have been done to prevent the attacks, or was it inevitable that al-Qaeda would breach US defences?
